BELOIT, WI. (July 12, 2021) — BHS will be offering the Pfizer vaccine for ages 12+ and older.

Vaccine Clinic For Those Eligible will be held on:
July 28, 2021 10:00am-3:00pm

NEW Location: Beloit Health System COVID-19 Vaccine Center 1904 Huebbe Parkway, Beloit.

The vaccine is being administered at no cost to the patient. Please bring your insurance card with you. A parent or guardian must accompany anyone under the age of 18 to sign the consent waiver form on-site for any individual under the age of 18 to receive their vaccine.

If you are an established patient at BHS, appointments can be made online using the adolescent’s MyHealth patient portal at beloithealthsystem.org/myhealth. Or, you may call our dedicated vaccine phone line Monday through Friday from 8am to 5pm at 608-364-5663 to make your appointment.
