Collaboration aims to address gap between access and availability of primary care, mental health services, and obstetrics care

 

Rockford, IL – August 2, 2017: Aunt Martha’s Health & Wellness, one of Illinois’ largest community health centers and most experienced providers of integrated mental health and primary care services, opened its on-site health center at SwedishAmerican Hospital on Tuesday, August 1.  The opening comes six months after the two organizations announced their intent to partner, a move which required federal approval from the Health Resources and Services Administration (HRSA).

 

“SwedishAmerican is extremely excited to partner with Aunt Martha’s and deliver high quality healthcare services to members of our community who perhaps needs it the most,” says SwedishAmerican President & CEO Dr. Michael Born.

 

“Too many of our residents, particularly those who are underserved and uninsured, now can gain access to better designed medical care. They will be set-up with a provider, having access to a medical home, rather than relying upon the emergency department for primary care.”

 

The new clinic, located in nearly 4,500 sq. ft. of newly renovated space on the Hospital’s third floor, will offer primary care services when it opens this week.  Women’s health services (obstetrics and gynecology), and mental health services (via telepsychiatry), are scheduled to begin in November.  Insurance enrollment assistance will also be available.

 

Aunt Martha’s clinic staff and care coordination team will work closely with SwedishAmerican’s emergency department (ED) to ensure that patients who do not have a primary care physician receive appropriate follow-up care in a medical home setting.

Aunt Martha’s clinic is located in Suite 3000 on the Hospital’s 3rd floor.  It will be open from 8:30 a.m. to 5:00 p.m. on Monday, Tuesday, Thursday, and Friday; from 10:30 a.m. to 7:00 p.m. on Wednesday; and from 9:00 a.m. to 2:00 p.m. on the fourth Saturday of each month.

 

The public is invited to attend an open house on Friday, August 18 at 11 AM. A short program, ribbon cutting and light refreshments will be provided.
